To recognize an outstanding volunteer from your organization and register for the banquet, submit the attached form below by April 11, 2013 to Dana Kominsky at the address to your right or dkominsky@nbn-nrc.org.
Recognized volunteers will be announced at the banquet and will receive a certificate of appreciation for their service to the community. There is no limit to how many volunteers you would like to nominate. We encourage representatives of the supporting organization to attend as well. Please enclose or email a photo of each volunteer you wish to recognize.

• We will have a full-course dinner starting at 5 pm catered by Heirloom Catering, a new caterer located in Muskogee!
  • If you have food allergies or special needs please contact Dana Kominsky.
    
• Organizations are asked to pay the cost of dinner if the volunteer plans to attend
• The cost is $20 per person or you can reserve a table of eight for $140.
• You can send us a check, pay at our office (cash, check, or credit card), or pay online below. If payment is not received 48 hours before the event you will be invoiced.
